When should a founder create the first real manager layer after crossing twenty employees?

A founder I advise has twenty-four employees and five informal team leads. Every issue still travels to the founder. What evidence should determine whether the company is ready for a formal manager layer?

The strongest signal is repeated coordination work requiring judgement across people and priorities. Define three decisions each lead will own and observe whether they resolve issues without forwarding everything upward.

We created manager roles too early once and only changed titles. The better approach was to identify natural work groups and assign output, capacity and feedback responsibility before the title.
